The Vantus Referral Process:
Sleep Study:
– Simply fill out the provided Rx requesting a sleep study and your patient will be coordinated with us and SleepTest.com to get their study performed in a timely manner.
– Note that the patient will have a telemedicine visit with a sleep physician prior to their study and then will receive their home sleep test unit delivered to their home. They will wear for two nights and then ship back in a prepaid shipping container. Once their results are read, they will have a follow up telemedicine visit with that sleep physician to coordinate them for CPAP or oral appliance therapy. We will keep you posted on their progress throughout the process.
Oral Appliance Therapy:
– Please fax over the following information to allow us to process your patient in a timely manner and to be able to submit the proper documentation for coverage through their medical insurance. If any of these are missing then there can be delays in getting your patient the therapy requested:
1. A copy of their diagnostic sleep study (within 1 year)
2. A signed and filled out Rx requesting OAT (template provided)
3. Any pertinent medical chart notes stating reasons for seeking OATor any documentation of CPAP non-compliance.
– You will receive updates about your patient’s progress.
– Once titration is completed (can take a few months after receiving their oral appliance), the patient will be referred to you to perform a titration sleep test to confirm efficacy.
– We would appreciate a copy of the titration study to be able to know what improvements the patient has gained.
